Fees 
Assessment and intervention fees vary depending on the assessment package selected and any additional therapy or support options required. 
 
All invoices for assessments will be issued at the time of booking. Payment in full is required no later than 48 hours prior to the scheduled assessment appointment via bank transfer (details provided on the invoice). 
 
All invoices for interventions will be issued at the end of each calendar month, and payment in full is required within two weeks of the invoice date. 
 
If payment is not received within the specified timeframe, we reserve the right to: 
 
Add on an additional 10% late fee per week the invoice is not paid 
Withhold delivery of sessions while retaining the full session fee; and/or 
Initiate legal proceedings for recovery of outstanding fees. 
 
Please note that any costs incurred by FunctionAbility Ltd in recovering unpaid fees (including charges from collection agencies, credit reference agencies, solicitors, or small claims processes) will be added to the final balance due. 
 
Additional Charges 
 
Where FunctionAbility Ltd is commissioned by a Local Authority (LA), Clinical Commissioning Group (CCG), or another organisation to deliver specific assessment or intervention services, any requests outside of the agreed commissioning terms (for example, additional reports, duplicate reports, meetings or supplementary documentation requested by parents, carers, or schools) will be subject to additional charges. These charges will be discussed in advance and invoiced accordingly. 
 
Therapists aim to maintain regular and constructive communication with parents and carers as part of the therapeutic process. However, if contact becomes excessive (for example, ongoing email, phone, or messaging contact beyond what is deemed reasonable or necessary for clinical progress), administration charges will be applied to cover the additional time required. Administrative time will be charged in 15-minute blocks at £25 per block. Families will be notified before any such charges are applied. 
 
Intake and Assessment Documentation 
 
Prior to assessment, intake forms and consent paperwork will be sent to you. These must be fully completed and returned, with a parent/guardian signature where applicable, at least 48 hours before the scheduled appointment. 
 
Failure to provide the required documentation within this timeframe may result in cancellation or postponement of your assessment, and the full session fee will still apply. 
 
Cancellations  
 
Sessions can be cancelled by email or telephone message. Appointments cancelled more than two days (48 hours) beforehand are not charged. 
 
Cancellations with less than 2 days’ notice or non-attendance will be charged at the full session rate. 

Record Keeping and Safety 

Details of and information shared within sessions will be kept confidential, though may be discussed (omitting identifying information) in peer supervision or where specifically agreed with you (ie. with your GP or other healthcare provider). 
 
The exceptions to this is the disclosure of information that indicates either the individual or another person are at risk of serious harm. In such instances where confidentiality may need to be broken, we will always endeavour to discuss this with you first, where appropriate. 
 
We process all client data in line with the guidelines set out by the Data Protection Act 2018. FunctionAbility is a registered practice with the Information Commissioner’s Office. All electronic client records and/or the devices on which they are kept are encrypted and/or password protected. On completion of our work together your records will be held in line with professional practice guidelines, after this your data and all electronic communications will be destroyed/erased. 
 
Parents/ carers are responsible for children / family members when attending the sessions. Safeguarding is taken very seriously at FunctionAbility, all staff take training annually. A copy of our Safeguarding Policy is available on request. Our Designated Safeguarding Lead (DSL) is Lucy White. 
 
The therapist will treat the child / adult at any venue with due respect and use appropriate language and dress code. It is expected that the adult/ individual will address the therapist with equal respect and wear appropriate clothing and use appropriate language. 
 
Any physical, verbal or emotional abuse towards any of our staff by the individual at any time will result in the termination of the session. Children whose condition involves potential for harm (either physical, verbal or emotional) either towards themselves or others should be notified to the therapist in advance. 
 
All therapists have a valid Enhanced Disclosure DBS document, which are viewable on the Update service.
