Partnerships, Referral Partners and Vacancies

Referral Partners

At FunctionAbility we work with a wide variety of referrers who benefit from accessing Therapy Services. This includes (but not limited to):

Care homes - this can include manual handling training, home hazard assessments, falls assessments, equipment assessment and review as well as support to promote independent living.

Supported living - this can include physical health support, mental health support, assess and support independent living skills, engagement in meaningful occupations and group sessions in a variety of areas.

Medical practitioners - this can include a variety of assessments including manual handling, physical health assessments and mental health assessments.

Case Managers - this can include assessment and intervention packages, Multidisciplinary team meeting attendance, training and support for individuals, care givers and professionals.

Local Authorities - this can include provision of assessment and report to reduce wait times, contribution to EHCP's, implementation of current OT provision on EHCPs and other documentation
